The VNA is seeking a QAPI Registered Nurse (RN) to perform quality assurance and performance improvement activities that focus on clinical management, risk management, staff performance, education and survey readiness with federal, state and accrediting agencies. Integrate and demonstrate the Core Values of the VNA in all aspects of work responsibilities.


  1. Assist with overall development, implementation and evaluation of the agency quality improvement (QI) plan.
  2. Maintain current knowledge of state and federal regulations and accreditation standards. Identify deviations and recommend methods to improve processes of care.
  3. Serve as a resource person on clinical, regulatory and reimbursement issues.
  4. Review concurrent and retrospective clinical records per the requirements of the QI plan or when needed for focused or other reviews. Follow up with the appropriate clinical manager to improve quality and adhere to regulatory requirements.
  5. Monitor quality improvement indicators including but not limited to: utilization review, client satisfaction, occurrences, infection control, complaints, potentially avoidable events and agency scorecard.
  6. Review, compile, summarize and report on QI activities and outcomes using the electronic medical record (EMR) and other software (e.g. SHP, Department of Elder Affairs) for QAPI or other VNA committee/department uses.
  7. Complete timely and appropriate reports or analysis in meeting with the requirements of the QI plan.
  8. Assess and evaluate the quality and appropriateness of care, admissions/discharges and utilization of levels and types of personnel insuring that client needs are addressed and services are utilized appropriately.
  9. Export HIS and/or OASIS data to CMS’s Quality Improvement and Validation System (QIES) Assessment Submission and Procession (ASAP) system. Monitor and review reports from the system as required.
  10. Participate in the development of agency parameters for appropriate documentation, clinical indicators and standards when necessary.
  11. Assume responsibility for follow-up on problems or questions related to client documentation.
  12. Collaborate and participate with other team members/departments in safety, risk management and infection control monitoring and other related activities as needed.
  13. Assist with ongoing associate education and new associate orientation.
  14. Attend clinical department or interdisciplinary meetings as needed.
  15. Attend mandatory staff meetings and in-service programs. Fulfill mandatory e-learning courses by the due date.
  16. Follow VNA safety policies and procedures and report accidents or unusual incidents immediately to your supervisor.
  17. Enter work time and mileage according to VNA timekeeping and mileage policies.


  • Active RN license in the State of Florida
  • Current Basic Life Support for Health Care Provider Certification/CP
  • Minimum of three years experience as a licensed professional
  • At least one year of experience in quality improvement in a health care setting
  • Home Health and/or Hospice experience preferred
