    Responsible for all administrative, clinical, organizational and operating/capital budgetary planning for all areas of responsibility. Responsible for the provision of quality and cost-effective services and systems; demonstrates and integrates the values of the VNA in the performance of duties. Role models intradepartmental collaboration with managers and staff in meeting the comprehensive needs of home health patients.

    Responsibilities

    1. As a member of the Senior Leadership Team, participates in the development of a multi-year strategic plan that includes assessing the needs of the customers to develop new Home Health Services to expand and enhance agency services.
    2. Develops and recommends changes to home health administrative policies and procedures. Interprets policy and communicates to supervisory and clinical staff, and others who may be affected.
    3. Coordinates and integrates all care team activities through regular meetings with department managers.
    4. Maintains appropriate levels of staffing and work schedules for the patient population served, including planning for times of emergency.
    5. Defines the qualifications and performance expectations of all positions in areas of responsibility.
    6. Develops proficiency in the agency electronic medical record.
    7. Assumes day-to-day responsibilities of Patient Care Manager during vacations or unplanned absences.
    8. In collaboration with the QI Department, maintains responsibility for assuring manager participation in and ongoing monitoring and identification of opportunities for improvement in care outcomes including internal and publicly reported outcomes, such as Medicare Home Health Compare and HHCAPS Patient Satisfaction reports. Develops action plans to address areas of opportunity and routinely reports on the status of plans.
    9. Actively participates in identifying and meeting annual senior manager/senior leader goals tied to agency CORE values.
    10. Routinely communicates essential announcements, department performance improvement activities and patient outcome data to all department staff through regularly scheduled meetings or other means of communication.
    11. Engages associates in discussion on patient care performance improvement activities.
    12. Approves budgetary expenditures within specific areas of responsibility.
    13. In conjunction with VP Professional Services, reviews monthly financial reports and implements recommendations for variance control.
    14. Determines order of succession for areas of responsibility.
    15. Interviews all clinical applicants for clinical positions or selectively delegates to a Patient Care Manager. Collaborates with VP Home Health in selection of department managers.
    16. Responsible for disciplinary action and termination decisions; maintains accurate and complete supporting documentation.
    17. Monitors PCM competency in coaching and evaluation of clinical staff.
    18. Monitors associate and client occurrence reports and assures appropriate documentation and follow-up with HR and QI departments as required.
    19. Reviews all client complaints; accountable to be the first responder to all client complaints not resolved by PCM. Collaborate with the Compliance Officer as indicated.
    20. Collaborates with the education department on the planning and execution of educational needs as identified. Participates in educating staff based in areas of expertise.
    21. Ensures compliance with applicable licensure, certification and JCAHO requirements. Oversees survey process during state licensure inspections.

     

    Qualifications

    • Current RN license in the State of Florida
    • 5+ years of home health experience with 3-5 years in a management role
    • Knowledge of regulations and Oasis
    • Bachelors degree required
