    The VNA is seeking a Patient Care Coordinator to act as a liaison between the field staff and physicians in obtaining plans of care and appropriate modified orders. The patient care coordinator will collaborate across all services lines and promote utilization of VNA services. Demonstrate VNA core values in all aspects of duties.

    Responsibilities

    • Receives information for/from field staff regarding patient care issues, prioritizes and reports these issues to the appropriate parties
    • Reviews findings with the admitting nurse for appropriateness of requested services
    • Recognizes problem areas and facilitates group problem solving
    • Coordinates care with all rehabilitation staff and obtains appropriate orders of care
    • Ensures that physician orders are signed in a timely manner
    • Attends regularly scheduled meetings with physicians and/or appropriate designees to review patient care issues, orders, 485s, 60 day summaries.
    • Participates in and conducts joint projects and presentation with other community resources to promote community awareness
    • Attends agency staff meetings and agency or non-agency continuing education programs as needed
    • Participates in the agency’s quality/process improvement processes as needed
    • Contributes to team effort by accomplishing related tasks as needed.
    • Helps to establish and maintain new and current accounts
    • Maintains weekly contact and visits with accounts on the Account List
    • Consistently meets or exceed monthly metrics
    • Provides educational material to patients as needed
    • Working with external clients to facilitate appropriate placement in services offered by VNA
    • Ensures the smooth transition of care between a facility and the services provided by the VNA
    • Responds to requests for care from referring sources in a timely manner
    • Obtains information to verify coverage and communicates that to the patient
    • Identifies both primary and secondary payer of services
    • Explains the function of the health care team members who will provide coverage for the patient
    • Coordinates the start of care activities with the Intake person and the Patient Care Manager
    • Functions as an expert resource for interpreting Medicare home health guidelines
    • Communicates pertinent information and instructions for specific physician orders to health team members including the completion of the interdisciplinary referrals
    • Provide information to physicians and the community on the benefits and alternative home health services that the VNA can provide
    • Document patient care coordinating activities according to departmental procedures
    • Makes appropriate referrals for patients not accepted for services
    • Assumes responsibility for administrative calls as assigned
    • Provides administrative support to the Physician Liaison.
    • Maintains patient care database by entering new information as it becomes available, verifying findings and reports and backing up data
    • Maintains patient confidentiality and respects patient rights

    Qualifications

    • LPN license in State of Florida
    • Minimum 1 year of homecare experience preferred
