    Assisting the Patient Care Managers with the expediting of information gathering for the purpose of order approval and patient care.


    • Assures that documentation supporting prognosis is received at the time of referral. Collaborates with the PCM, VP and Medical Director as needed regarding issues of questionable eligibility criteria or medical interventions
    • Serves as a resource for clinicians and assist with answering of phones, calling of doctors office, faxing of lab orders, gathering of referral documentation
    • Collaborates effectively with the Home Care PCMs
    • Provides mentoring and support for field staff, serving as a clinical resource. Completes annual performance evaluations with input from PCMs  
    • Enters interaction and/or activity with patient, family, etc. into HomeCare HomeBase coordination notes to ensure continuity of care for the patient. Provide information on patient condition and interventions provided. 
    • Attends on an as needed basis, interdisciplinary group (IDG) meetings to stay abreast of challenges facing hospice staff once admitted to program and to determine how referral specialist can collaborate with team.
    • Participate in ongoing process improvement initiatives. 
    • Other duties may be assigned to assist in providing Hospice care to patients and families.


    • Associate's degree (A. A.) or equivalent from two-year College or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
    • Proficiency in typing and Microsoft office suite is required
    • General knowledge of computer, health care, and medical terminology is highly desired
    • Customer service focus with professional telephone etiquette is required
    • Prior Home Health experience preferred
