    The Medical Records Specialist will maintain, retain and protect agency medical records. Maintain, retain and protect home health and hospice patient medical records consistent with Federal and State regulations, VNA policy and procedures, and The Joint Commission (TJC) standards. Perform role with supervision by Director of Quality Improvement. Demonstrate VNA core values in all job duties.



    1. Scan and upload documents into clinical record systems.
    2. Ensure clinical record systems are maintained in compliance with state, federal and TJC regulations.
    3. Maintain comprehensive working knowledge of federal, state and TJC documentation regulations and serve as a resource for appropriate organization personnel.
    4. Protect clinical records through the establishment and implementation of control procedures for all open and closed records.
    5. Protect the confidentiality of patient and organization information through effective controls and direct supervision of clinical records.
    6. Forward copies of clinical records to authorized users according to policy.
    7. Complete medical records requests when needed for clinical, quality or billing audits and reviews.
    8. Complete an administrative record audit following patient discharge.
    9. Oversee the use of the clinical records system and maintain a comprehensive working knowledge including upgrades and enhancements.
    10. Monitor aged unsigned physician orders and submit additional requests for signed orders when overdue.
    11. Plan, prioritize, organize and complete work to meet timeframes and objectives. Request additional administrative or volunteer assistance as needed to meet required timeframes.
    12. Safely transport medical records and related material to VNA partners in the local area
    13. Comply with HIPAA and other VNA policies and procedures
    14. Coordinate transfer to off-site storage facility and recommend destruction of records per guidelines.
    15. Maintain a working knowledge of additional software systems utilized including upgrades and enhancements.
    16. Assist on special projects and other duties as needed.
    17. Comply with all VNA policies and procedures, including safety expectations


    • High school diploma or equivalent certification required
    • Minimum of one to two years’ experience in healthcare office administration or medical field:
    • Medical records background/experience with home health and/or hospice experience preferred.
    • Electronic Medical Record (EMR) ; Home Care Home Base (HCHB) preferred.  
    • Valid Florida driver’s license and driving history with no major infractions
    • Must be at least 21 years of age due to driving requirement
