Job Summary: Promote community health and wellness through participation in health screenings and attending screening clinics and health fairs; administer vaccines at vaccination clinics and during home visits to homebound individuals throughout Indian River  county. Responsibilities

Essential Job Duties/Responsibilities

  • Health screenings and health screening clinics
  • Conduct blood pressure, blood sugar, and other health screenings, as assigned.
  • Perform screenings according to department policies and procedures.
  • Educate clinic attendees about the importance of maintaining a healthy lifestyle; disease prevention; and the role of regular monitoring and testing, as appropriate.
  • Document all referrals on referral forms for submission to management.
  • Return clinic sign-in sheets and referral forms to management at end of each clinic.
  • Store and maintain clinic supplies in assigned VNA carry bag at VNA (it is not permissible to store VNA supplies in an associate’s or contractor’s home).
  • Immunization clinics
  • Clinic Site Manager (RN only)
  • Works with management in advance of assigned clinics to coordinate pick-up of supplies and vaccines with return of supplies and unused vaccines on same day of clinic(s).
  • Maintains communication with main points of contact for public and private clinics.
  • Arrives 15-30 minutes prior to clinic start time depending on size of clinic, working with site contact to determine location and clinic flow.
  • Sets up posters/signs to advertise clinic availability (not at roadside).
  • Supervises and assists in work area setup, including seating area for post-vaccine observation.
  • Acts as resource for clerical personnel including verification of clinic worksheet data. Verifies vaccine logs are complete.
  • Administers vaccines adhering to universal precautions and VNA policies and procedures.
  • Consents each patient using VNA-approved consent form, ensuring accuracy of information.
  • Document vaccine dose and injection site and sign/date consent form.
  • Arrive at clinic 15-30 minutes prior to start time depending on size of clinic.
  • Report to clinic site manager and assist in work area setup.
  • Participate in work-area clean-up upon clinic completion.
  • Represents VNA at Health Fairs to promote VNA services.
  • Conducts home visits to homebound individuals to administer flu, COVID-19, or other vaccines per VNA policies and procedures

Qualifications

Skills/Abilities

  • Excellent communication – verbal, written, and listening

  • Infection control and clinical competencies required

 

Qualifications: To perform this job successfully, an individual must have the following.

  • Active RN or LPN license in the State of Florida
  • Current Basic Life Support for Health Care Provider Certification/CPR
  • Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing. Vaccine administration experience helpful.
