The Weekend Overnight Call RN will be responsible to triage and ensure clinical expertise, after business core hours, for VNA patients in home care.

Responsibilities

  • Assume primary responsibility for after hours call for VNA paitents in home care
  • Provide after hours care for all home health patients and /or family within their home setting with a focus on satisfying the care plan, reported symptoms and/or educating the patient and /or family as needed.  
  • Facilitating after hours admissions
  • Makes on-site visits as needed.
  • Telephone Triage and patient education
  • The quality of care will be consistent with the Standards of Practice determined for the home health program.
  • Act as a liaison between VNA and patient/families/physicians/referral sources after hours responding to calls in a timely manner.
  • Communicate and act in a professional and supportive manner.
  • Report/communicate to the respective clinical team at the completion of each call assignment to promote a smooth transition of care.
  • Documentation in electronic medical record system is completed within agency guidelines and to meet regulatory requirements.
  • Review and approve start of care (SOC) of clinical RN staff in order to expedite the scheduling of patients for the next day.
  • Review and approve IRC HCHB workflow to support clinical operations

Qualifications

  • Florida RN license required
  • Previous home health experience is preferred.
