The VNA is seeking a Clinical Manager (PCM) for our Home Health Division. In accordance with regulatory requirements and the mission and vision of VNA of the Treasure Coast, this position is responsible for coordination and management of patient care and the supervision of clinical associates to ensure that care and services are delivered appropriately. Demonstrate Core Values in all duties. Perform duties under general supervision of Director or VP of Clinical Services. Per regulation, Clinical Manager (PCM) may functions as the Director of Nursing.

Responsibilities

  • Plan, organize, direct and evaluate clinical service delivery to continuously improve the quality, coordination, and effectiveness of service delivery.
  • Understand and communicate policies, procedures and clinical protocols to assure that reporting staff have a working knowledge of agency practices and that care is delivered in accordance with agency expectations.
  • In cooperation with the VP of Clinical Services  or Director of Professional Services Home Health and the Regulatory Compliance Quality function, actively participate in the oversight of data collection (both internal and publically reported) measuring clinical performance and patient outcomes. The Patient Care Manager is responsible to contribute to corrective action plans when negative trends are reported. The Clinical Team Supervisor (PCM) utilizes this information in the completion of periodic shared field visits and annual performance appraisals
  • Participate in developing programs to improve clinical, system and fiscal outcomes.
  • Demonstrate working knowledge of Medicare/Medicaid Conditions of Participation and Joint Commission accreditation standards applicable to home health.
  • Demonstrate working knowledge of the managed care industry.
  • Ensure compliance with applicable legal requirements, standards, policies and procedures.
  • Supervise associates in the home health nursing department and/or team. Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training; planning, assigning, and directing work; appraising performance; rewarding and disciplining associates; addressing complaints and resolving problems.
  • Demonstrate clinical and management skills including hands-on care, nursing judgment, organization, leadership, problem solving, critical thinking, verbal/non-verbal communication and conflict resolution
  • Mentor nurses in development of their management skills i.e.,critical thinking, time management, and problem solving.
  • Effectively use the agency’s software programs to plan, coordinate and deliver patient care including processing physician orders, plans of treatment, scheduling, review and oversight of OASIS and other applicable assessment tools
  • Work collaboratively with other departments in the coordination of care and services.

Qualifications

  • Current RN license in the State of Florida
  • Minimum of three (3) plus year’s experience as an RN including management experience and (1) year in home health.
