The Visiting Association Nursing of Treasure Coast is seeking a Bereavement Counselor to work with our Hospice Department.

 

Responsibilities

1. Completes bereavement assessments as needed.

2. Reviews all staff initial bereavement assessments and progress notes of survivors noting changes in risk level and intervenes appropriately when following up with BVT special referrals.

3. Establishes a plan of care based on risk level and initiates same.

4. Works with high risk survivors, identifying issues and intervening appropriately.

5. Refers families and/or invididuals to appropriate community resources, both locally and nationally.

6. Prepares and keeps current documentation of survivors' cases for the record.

7. Facilitates support groups, provides bereavement services to the community, and individual counseling within the school system.

8. Assists in screening/interviewing prospective bereavement volunteers and works closely with volunteer coordinator. In the absence of the bereavement coordinator assists in overseeing bereavement volunteers tasks and responsibilities.

9. Participates with training of prospective volunteers, staff, etc., in conjunction with the Bereavement Coordinator.

10. Provides consultation to members of staff, families, community agencies, and other persons seeking guidance with grief issues, as time permits.

11. Plans and develops new bereavement programs in conjunction with the Bereavement Coordinator. Works with Bereavement Coordinator in carrying out all bereavement programs; i.e., Camp Chrysalis, Memorial Service, NHF Teleconference, Staff Memorial Service, etc.

12. Participates in in-service programs.

13.Participates in case conferences, in services and QA activities, and community health projects, community education and speaking engagements.

Qualifications

  • Master's degree in Social Work required.
  • Experience with grief and loss counseling for children and adults, individually as well as support groups required.
