The coordinator is a support role within a very active Community Health, Education and Wellness program providing coordination for VNA’s community health initiatives including: a mobile health clinic; vaccine and health screening clinics; Together for Health group; Positive Change Program, and other programs and projects, as assigned. The primary focus is to work directly with the Director of Community Health, Education and Wellness to ensure efficient operations including, but not limited to, community outreach, scheduling, meeting minutes, administrative duties, and data entry and analysis. Responsibilities

 

Provide coordination for the department’s community health initiatives including the mobile health clinic, vaccine program, Together for Health group, Positive Change Program, and other programs and projects as assigned with the primary focus to work directly with the Director of Community Health, Education and Wellness to ensure efficient operations, including outreach, scheduling, meeting minutes, administrative duties, data collection and analysis. Responsibilities include:


  • Program data entry, data management, and analytics for the community health initiatives including mobile health clinic, vaccine program, Together for Health group, Positive Change Program, and other programs and projects, as assigned.
    • Management of the program’s health screening tools.
    • Scheduling and coordination of multiple activities for community health initiatives including mobile health clinic, vaccine program, Together for Health group, Positive Change Program, and other programs and projects, as assigned.
    • Maintain patient information, develop schedules and input data in Florida Shots for vaccine program, Positive Change program, and other initiatives of the department.
    • Office support including copying, filing, and document scanning; assembly of patient binders and other program materials; management of patient data binders; appointment scheduling and follow-up.
    • Assistance with development of promotional materials for department initiatives.
    • Management of the program’s health screening tools including data entry and analytics.
    • Copy editing and proofreading.
    • Assist with development of PowerPoint presentations.
    • Generate memos, emails and reports, as directed.
    • Plan, organize and perform daily tasks and assignments that support the Community Health, Education and Wellness department.
    • Provide customer service as needed; respond to inquiries with accurate and timely information.
    • Gather facts and information related to programs and communicate effectively so the team can respond timely and appropriately.
    • Assist with enhancing efficiency with procedures, processes and services delivered by staff and department.

 

Qualifications

 

  • Public Health background preferable.
  • Associate degree or above preferable.
  • Previous experience in non-profit management preferred with at least 1-2 years in a business office environment. However, demonstrated dedication to public health and enthusiasm for the work is most important.

 

Skills/Abilities:

 

  • Excellent professional communication skills via phone, e-mail, and mail.
  • Must be proficient in Microsoft Office with emphasis on Word, Excel, SharePoint and PowerPoint
  • Must be willing and able to learn other systems used by department and become proficient
  • Exceptional writing skills
  • Proficiency in time management
  • Must be able to multitask as necessary
