The Visiting Nurse Association of the Treasure Coast is seeking a Director of Nursing for our Home Health division. In accordance with Federal and State regulations, this position serves as an Alternate Administrator for all home health care or related health care program, and as needed, if designated as the Administrator. Follow regulatory requirements and mission and vision of VNA of the Treasure Coast.  A licensed clinical professional in accordance with 484.105(c): Conditions of Participation. Assumes authority, responsibility, and accountability for the delivery of clinical services for all Home Health Services delivered to patients and their families.  In collaboration with the Servant Leadership Team allocates organizational resources in an economic and efficient manner to enable each patient to achieve their optimal quality of life including their physical, mental and psychological health.  Leads all departments who offer nursing and therapy services that represent the full continuum of care that serve the patients and their families.  Also responsible for collaborating with other departments that are engaged in the practice of governance and compliance including relationships with all accreditation agencies. Coordinates all administrative functions and represents the interests of the organization with all stakeholders.  Available during all operating hours and after hour rotation to ensure that patient care and services are continually met and delivered appropriately. Reports to Executive Director, IRC Home Health with some supervision.  Incorporate and demonstrate the Core Values of the VNA – Integrity, Collaboration, Accountability, Respect, and Excellence at work and whenever representing the VNA. Responsibilities

  1. Develops, maintains, and implements clinical policies and procedures that conform to current standards of nursing and therapy practices, facility philosophy, and operational policies while maintaining compliance with state and federal laws, regulations and accreditation standards. 
  1. Ensures that care and service delivery meets patient needs and is in compliance with professional practice standards and the Conditions of Participation.
  1. Directs, coordinates and supervises all clinical functions and assumes primary responsibility for clinical services provided by Agency.
  1. Communicates and interprets policies and procedures to clinical staff and maintains metrics to measure quality and patient satisfaction and develops strategies to ensure that policies and procedures are updated and effective at providing high quality and patient satisfaction scores.
  1. As needed, works with the Physician Liaison team to assure that all admission decisions are appropriate.   
  1. Participates with Patient Care Managers in daily team meetings to discuss patient status, census changes, staffing challenges, patient complaints and physician concerns. 
  1. Ensures efficient and effective operation of the geographic PODS comprised of Patient Care Manager, Intake, and Scheduler.
  1. Assures that there is a system to measure the performance and competency of clinical staff and the evaluation is done annually.
  1. Takes corrective action when necessary when performance and competency of staff is below expectations according to organizational policies. 
  1. Ensures delivery of compassionate quality care by supervising the provision of care by clinical staff while providing the full range of services to patients through appropriate levels of staffing.
  1. Demonstrates knowledge of and application of Key Clinical Quality Indicators, and proactively monitors and implements systems to achieve company goals. 
  1. Exercises overall supervision of patient assessments and care plans. 
  1. Oversees or is responsible for all of the annual obligations of clinical staff members to maintain certifications, licenses and educational requirements. 
  1. Reviews daily reports that monitor and ensure timely and effective responses to patient conditions including any injuries, falls, medication errors or conditions that require oversight.
  1. Collaborates with physicians, consultants, community agencies, and institutions to improve the quality of services to patients and their families and resolves identified problems promptly.
  1. Oversee and supervises the development and delivery of clinical orientation program to equip patient care staff with knowledge and skills to provide quality care and respect for patients and their families. 
  1. Implements in-service and continuing education programs to meet education and training needs of associates including conducting shared visits with clinicians as needed.
  1. Responsible for the implementation and growth of the Telehealth program.
  1. Assures Quality of care by rounding with patients, families and staff 
  1. Is an active member of the QA Committee.
  1. Coordinates and/or develops on-going QA activities for clinical services to monitor clinical compliance with standards and regulatory requirements through rounds, interviews, and record reviews. Compiles summary of findings for the Quality Assurance committee.
  1. Works closely with the Director of Quality to ensure quality measures are met.
  1. Participates in budget development.
  1. Provides leadership and preparation for scheduled and unscheduled accreditation surveys including ACHA and JCAHO.

Qualifications

  • Must possess a current Florida license as a RN and graduated from an accredited school of nursing. Ten years of professional nursing experience in the Home Health care setting is preferred, and a minimum of five years’ experience in nursing administration or management.
  • Must possess and demonstrate a high degree of leadership, organizational ability, and communication skills.
  • Knowledge of Home Health care management, policies, budgeting, personnel management, and census development is required.
  • Current CPR certification is preferred.
