Overview

 

The Visiting Nurse Association of the Treasure Coast is seeking a  Patient Care Manager for Hospice to be responsible for implementation, coordination and supervision of all aspects of the delivery of palliative care to patient/family services, consistent with VNA Hospice of IRC policies. Supervises direct patient care to assure that physical, emotional, spiritual and financial needs are addressed.

 

Responsibilities

 

  • The PCM is responsible for the planning, coordination and delivery of care to patients and their families.
  • Assigns each patient/family to the appropriate associate (s) implements and monitors compliance with agency policies and procedures. 
  • Provides for and conducts nursing meetings for information and staff support as indicated.
  • Participates in the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
  • Provides verbal and written performance evaluations for assigned nurses and HHA's.
  • Participates in the development of, implements and documents written policies and procedures to provide staff with methods of meeting collective and individual responsibilities as outlined and assigned in the patients' interdisciplinary plan of care.  These policies shall at a minimum include methods of controlling symptoms of terminal illness together with methods of evaluating and studying such methods and ways of teaching patients and family members those hospice skills necessary to promote the patient/ family relationship and enhance the independence of the patient/family.
  • Serves as a role model to demonstrate high standards for nursing practice and fosters professional development in staff.
  • Collaborates with medical director as well as with other outside agencies and department within VNA.
  • Coordinates the timely processing and response to referrals, schedules and assigns admissions.
  • Using an approach that focuses on excellent customer service, responds to phone calls from patients, families and facility staff, providing education and support as indicated.
  • Utilizes excellent critical thinking and assessment skills in responding to patient needs, assigning nursing visits as indicated to meet the needs of the patient/family.
  • Collaborates with Hospice PCMs, Scheduler and Hospice House Manager to assure seamless delivery of care over the weekend hours.
  • Assures a timely response to patient needs during evenings and nights.
  • Provides mentoring, education and support to staff.
  • Collaborates effectively with Hospice House staff to assure timely transfers and admissions.
  • Possesses excellent communication skills in the coordination of care with referral sources, DME and medication providers, facility staff and hospice physicians.

 

Qualifications

  • Current RN license in the State of Florida
  • Minimum of 2 year experience as an RN, Hospice experience preferred
  • Prior management experience
