The VNA is seeking a Therapy Manager in our Indian River County Home Health division. The therapy manager will assume primary responsibility for the supervision and evaluation of all physical, occupational and speech therapists, therapy assistants, as well as the therapy interdisciplinary team coordinator/Scheduler. Build and maintain relationships between VNA ,physician offices, community and community health organizations. Report to the Senior Director of Nursing.

Responsibilities

  • Responsible for program development and implementation, training of appropriate therapists as champions and collaboration with community partners,
  • Leads initiatives and acts as a business partner with community referral sources developing strong business relationships.
  • Works collaboratively with VNA service line leaders.
  • Provides insight to the leadership team. 
  • Oversees the rehab and restorative patient census. 
  • Supervises all therapists in the development, implementation, coordination and evaluation of rehabilitation care plans.  Sets standards of care for Rehab therapy services, ensures adherence to standards by therapy staff, and delivery of appropriate POC.
  • Guides therapists and therapy assistants in the adequate documentation of all pertinent information in the patients record.  Supervises therapists in the documentation needed by third party intermediaries for reimbursement. 
  • Participates in QI processes; reviews documentation, evaluates clinical practices and assists in the development review of policies and procedures for care delivery as requested. 
  • Demonstrates effective leadership skills of delegation, organization, problem identification and solving skills.   
  • Collaborates with Patient care managers to ensure interdisciplinary care delivery, to include development of formalized case conferences. 
  • Maintains effective communication with staff, physicians, patients, families and community agencies. Acts as a liaison and consultant to staff and physicians as needed. 
  • Ensures adequate therapy coverage. Coordinates rehab services with other disciplines. 
  • Interviews, makes hiring recommendations, and orients new therapists to the rehab processes. 
  • Assists in preparation of job responsibilities, prepares annual evaluations, makes shared visits with therapy staff.  Provides in-home supervision of rehab staff. 
  • Functions as a role model in home health care, demonstrating competency in clinical judgement, selected skills and appropriate knowledge base.  Remains aware of current research and implements relevant findings as applicable.
  • Accountable for productivity of therapists and timely completion of paperwork and the exportation of point of care documents from lap top computers. 
  • Conducts therapy weekly meetings or as determined ,participates in monthly all staff meetings, and supervisory/administrative committees as needed. 
  • Demonstrates knowledge of Agency policies and refers to them as a basis for decision making for clinical and management issues.  Teaches policies and procedures to therapy staff. 
  • Plans, conducts, and evaluates continuing education programs for therapy staff.   
  • Assists in developing competencies for staff training and assessment of skills. 
  • Assists in development and training of therapists related to computer pathways and documentation. 
  • Demonstrates knowledge and adherence to safety and legal requirements. Initiates a risk management process for the therapy department.

Qualifications

  • Current PT license in the State of Florida
  • Minimum of 3 years experience in the field of PT
  • Minimum of 2 years experience in the Home Health field
