Overview

The Visiting Nurse Association of the Treasure Coaast is seeking a Hospice Home Admission Manager.  The Admissions Manager oversees the customer flow through the hospice referral and admissions process, while actively supporting marketing and customer relations initiatives for the program.  This role supervises the Admissions team, ensuring high levels of patient, family, referral source, and facility satisfaction. It is essential to maintain a positive public image, achieve and exceed hospice census goals, and enhance market penetration.

Responsibilities

 

   Lead team to identify patient/family needs and provide admission services per federal and state guidelines.

· Oversee initial patient/family education on hospice election, services, and coverage related to the diagnosis.

· Coordinate with attending physicians, Interdisciplinary Group (IDG), and other agencies to ensure optimal patient/family care.

· Maintain regular communication with the Hospice Admissions team to review caseloads.

· Address unusual or potentially problematic patient/family issues with the Hospice Home care team.

· Collect data on factors that support hospice eligibility, including physical, psychological, social, and spiritual aspects.

· Update and review patient medical records and gather input from team members on patient care plans.

· Meet regularly with Hospice nursing staff to discuss issues, provide support, and promote professional growth.

 

Qualifications

· Current RN license in the State of Florida, BSN preferred, sales experience a plus

· Minimum of 2 years’ experience as an RN in Hospice Care, management experience required

· Extensive knowledge of Hospice Conditions of Participation with Medicare preferred

· Certification in Hospice and Palliative Care a plus
