The Visiting Nurse Association is seeking a Clinical Dietician to assess client nutritional status according to physician orders and provide consultation to the staff on high-risk nutritional clients, complicated diets, or on nutrition related knowledge deficit client cases. 

Responsibilities

 

1. Assess client nutritional status according to physician orders.

2. Has comprehensive and thorough understanding and knowledge of dietary care.

3. Provides consultation to the staff on high-risk nutritional clients, complicated diets, or on nutrition related knowledge deficit client cases.

4. Provides educational in services and consulting to clinical staff members as requested by staff development.

5. Completes documentation in HCHB related to patient care, phone contacts, case conferences and follow ups.

6. Submits all documentation same day as service delivered.

7. Attends meetings as requested.

8. Maintains ethics, rights and confidentiality of all client and company policy and procedures.

9. Maintains verbal and written communication with the client's Patient Care Coordinator regarding assessments, progress of client and collaborates nutritional assessment  needs into the plan of care/treatment.

10. Consistently maintains a good rapport and instructs clients/care givers, professional medical staff and physicians on nutritional health practices which will promote well being and self care practices.

11. Assists in Coordinating adequate nutritional supplements, nutritional sources and nutritional related supplies in the home environment for the client.

12. Observes uniform dress code and wears I.D. badge when performing home visits.

13. Knowledge of and follows chain of command when performing work related duties.

Qualifications

  • Registered Dietician Certificate in the state of Florida
  • 1-2 years related experience and training
  • Bachelor’s degree (BA) as a Registered Dietician required
