• Initial assessment of the client and supervisory visits as defined or as needed.
• Development and evaluation of the plan of care in partnership with client, representative, if any, caregiver(s), and physician if indicated
• Client, caregiver and family education and counseling
• Assume primary responsibility for a client caseload, to include completing new admissions timely, keeping documentation up to date, coordinating all services and collaborating with the healthcare team.
• Communication with team members for the plan of care as well as other health care practitioners, as appropriate
• Use, store, and maintain equipment and supplies safely and appropriately.
• Attend case conferences with supervisors to discuss new admissions, potential discharges.
• Utilize inter-agency and community resources to ensure continuity of care.
• Maintain and update client records accurately and clearly, reflecting changes as they occur.
• Complete care documentation accurately and timely within the established time limit for timely processing for continuation of plan, billing, and other VNA and department requirements.
• Follow HIPAA guidelines and maintain all protected health information (PHI) as confidential.
• Perform all nursing duties in accordance with the Florida Nurse Practice Act and comply with VNA policies and procedures.
• Attend mandatory staff meetings, team meetings, and in-service programs, and fulfill mandatory e-learning courses by the due date.
• Participate in chart audits and QA activities.
• Follow VNA safety policies/procedures and report accidents or unusual incidents immediately to the supervisor and Human Resources.
• Participate in candidate interviews, selection, onboarding, performance management, payroll, ongoing training, shared visits, and associate morale.
• Assist in orientation of new associates.
• On call rotation as well as holiday coverage as determined to best serve clients
• Provide instruction and supervision of CNA`s and HHA`s on a timely basis, and in accordance with agency, State, and federal regulations.
• Conduct regular education assessments for caregivers to ensure they are knowledgeable about current best practices and protocols.
• Develop and implement training programs to address any gaps in knowledge or skills identified during assessments.
• Track the progress and completion of training programs to ensure caregivers are meeting the required standards.
• Include annual fit testing for caregivers to ensure they are properly equipped to handle their duties safely and effectively.

• Active RN license in the State of Florida
• Current Basic Life Support for Health Care Provider Certification/CPR
• Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing
• Current and valid Florida driver’s license
• Current automobile insurance per VNA requirement
