The VNA is seeking a full time Hospice Admissions Coordinator. This position will be responsible for coordinating the admission process from time of referral to scheduling of start of care. Shares with the hospice scheduler the overall office/secretarial duties associated with the Hospice department. Acts as the first line responder to inquiries regarding access to and education regarding hospice services. Assists the Hospice VP with administrative functions as indicated.

 

Responsibilities

  • Coordination of the admission process to include processing referrals in HCHB, identification and contact of next of kin, scheduling visits and assigning admission staff as well as facilitation and acquisition of consents when necessary.

 

  • Coordination of staffing for Continuous Home Care, contacting agencies for shift coverage, processing and verifying coverage for billing, and maintaining CHC records.

 

  • Responsible for hospice contract maintenance. Insures contracts are current and obtains necessary certifications as needed. Preparation of one time supplier contracts, and hospice patient travel contracts.

 

  • Organization and maintenance of Special Needs/Emergency Preparedness roster. Provides updated information to EMS. Ensure applications and HCHB shelter status are accurate.

 

  • As part of clinical support, responds to outside calls from patients/families, contracted providers and staff, transferring to the appropriate resources as indicated.

 

  • Prepares department purchase orders for approval, entering information into Web purchasing

 

  • Organization of department meetings. Schedules meetings, assigns room resources, facilitates catering, notifies staff and takes minutes.

 

  • Transcription of hospice meeting minutes as needed.

 

  • Maintains the on call schedule

 

  • Prepares and disseminates the daily and weekend report assuring that it is up to date and accurate

 

  • Provide secretarial support for the hospice home team and Vice-President.

 

  • Acts as back up and vacation coverage for scheduler.

 

  • Coordinates special projects as needed. Prepares reports on efficiency, Length of Stay, Census, Discharges and Non-Admits.

Qualifications

High School diploma or GED equivalent, previous recent healthcare experience required. Associate’s degree (A.A.) or equivalent from two-year college or technical school preferred.

Customer service focus with pleasant phone manner, self directed, highly organized, calm, focused, mature professional required. Prior hospice, home care, long term care or hospital experience preferred.
