The Visiting Nurse Association is seeking licensed CNAs and certified HHAs to work with Private Care serving multiple clients in a day to provide optimal, high quality care in their homes. A typical day will include physical and emotional care as well as home safety, fall prevention, fire prevention, and nutrition issues in order ensure client safety, comfort, and security. Candidates for this position must be highly motivated, dependable, and must be able to work independently, to carry out the specific duties assigned by the LPN or RN in charge, maintain accurate documentation regarding the client's condition and progress, and keep records of all the tasks and services performed in a patient's home. Responsibilities

  1. Perform ADL and personal care activities for the client according to the written assignment by the RN. Per State regulations the CNA/HHA may:
    • Apply external catheters
    • Give or assist with personal care, e.g., care of mouth, skin, hair or other care as assigned by the professional nurse
    • Assist to bathroom, commode or in using bedpan or urinal
    • Assist with transfers and ambulation with or without an assistive device
    • Assist with prescribed Range of Motion (ROM) and prescribed exercises which the client and HHA have been taught by the appropriate professional personnel or written instructions left in the home
    • Assist the client with eating, meal preparation including special diets prescribed for the client
    • Assist the client with relearning household skills
    • Take temperature, pulse, respiration and blood pressure-
    • Perform Foley catheter care
    • Reinforce dressings, if needed, and contact RN Nurse Case Manager, Patient Care Manager or Director
    • Document personal care given on client's daily record
    • Assist client with self-administered medications with completion of AHCA approved course for Assistance in Self Administration of Medications
    • Assist with emptying colostomy bag if taught and supervised by RN
  1. Provide safe, competent care to promote client health, hygiene, and comfort
  2. Use/store equipment and supplies in a safe, appropriate manner; Follow VNA safety policies/procedures. Report accidents or unusual incidents immediately to the supervisor and Human Resources
  3. Provide light housekeeping tasks such as meal preparation, dishwashing, laundering client’s garments, linen change, and dusting and vacuuming client’s room. Provide pet care if needed.                                                                                                                     
  4. Private duty aides: Provide transportation for the client to physician appointments, errands, outside activities etc., as required   
  5. May assist with installation and troubleshooting of telehealth devices in the patients’ homes
  6. Follow HIPAA guidelines and maintain all protected health information (PHI) obtained in the course of work as confidential and, as such, should not be a matter of discussion with co-workers or others outside of the Agency
  7. Assist the client to reach maximum potential with mobility and activities of daily living under the direction of the RN or Therapist
  8. Document client’s condition in EMR or telephony reports to ensure continuity and quality of care and maintain clinical records according to agency guidelines
  9. Observe and report client physical and mental changes to RN Nurse Case Manager, Patient Care Manager or Director
  10. Report accidents or unusual incidents immediately to the Director, Patient Care Manager or RN Nurse Case Manager
  11. Comply with VNA policies and procedures
  12. Follow safety practices, procedures and policies.  Report client safety hazards to supervisor.  Report work-related injury/illness immediately to Director and Human Resources
  13. Attend required meetings and complete mandatory education by the department deadline

Qualifications

  • Valid CNA License or HHA Certificate (required)
  • Current CPR certification

  • Minimum six months work experience in home health, hospice or acute/long term facility

  • Must have a Current and valid Florida driver’s license

  • Current automobile insurance per VNA requirement
