Overview

The Visiting Nurse Association of the Treasure Coast is seeking a Per Diem RN to work in our Hospice House. Situated on a 4.2-acre secluded site, the Hospice House is surrounded by lush foliage, a pond with waterfall, a gazebo and numerous meditation areas. The VNA Hospice House, a unique blend of beauty and function, is the only in-patient hospice facility in Indian River County, for those facing life-limiting illnesses. Each of the 12 private bedrooms is completely handicapped accessible and has a private bath, sleeping accommodations for overnight family visitors, and screened patio with doors wide enough to allow bed access. Flexible shifts days or nights available. Must be currently licensed as an RN in the state of Florida. Must have at least two years acute care or Hospice experience.

Responsibilities

1. Identify patient/family needs and provide supportive care in accordance with the attending physician's orders and plan of care and VNA Hospice policies and procedures.

2. Assumes primary responsibility for a patient/family case management that includes the assessing, planning, implementing and evaluating phases of the nursing process.

3. Initiate communication with attending physicians, other Hospice staff members and other agencies, as needed, to coordinate optimal care and use of resources for the patient/family.

4. Maintain regular communication with the Hospice House Manager regarding patient acuity.

5. Inform the Hospice House Manager of unusual or potentially problematic patient/family issues and other means.

6. Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

7. Maintain up-to-date records and seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.

8. Meet regularly with Hospice nursing staff to review problems or unique issues, share professional support, and exchange feedback aimed toward enhancing professional growth.

9. Share in providing 24 hour, seven-day week coverage to patient/families.

10. Provide appropriate support at time of death and period of bereavement.

11. Provides supervision to CNA/HHA and prepares assignments.

12. Maintains the most up-to-date knowledge of symptom management.
13. Maintains compliance with policies and procedures and regulatory agencies.

14. Ability to work with diversified groups of people.

15. Attends and participates in meeting pertinent to job responsibilities.

16. Participate in VNA Hospice orientation and in-service training programs for professional staff.

17. Participate in agency and community programs as requested, to promote professional growth and understanding of Hospice care.

18. Promotes customer service.

 

Qualifications

  1. Must be currently licensed as an RN in the state of Florida
  2. Hospice experience required
