The VNA is seeking a Per Diem Evening Skilled RN to work with our Care at Home team. Primary responsibility for intermittent nursing visits to acute care patients in their home environment. Collaborates with lead command RN, assessment of clients (patients), from start to end of each visit. Assess client environment including family, as part of the ongoing assessment that includes communication and education about care and treatment for the client by utilizing family members or designated persons. Provide direct patient care and provide comprehensive service, initiating interventions, evaluating and documenting outcomes, and coordination of care through the interdisciplinary Cleveland clinic team. Initiate and maintain communication with physicians, immediate manager and community partners and vendors as needed. Work independently with some supervision by a Clinical Manager/Patient Care Manager (PCM).
